It is Golf ScoreCard Lite and can be found in the Entertainment category under Applications.

GolfScoreCard is an app that works like a Golf Scorecard. It allows you to put your scores from your game, as you play or afterward and save those games to be viewed later. It allows you to stop and continue the game. You can also view the holes, pars and scores on a ScoreCard and save the data to an image or text file.
